Darren Ferguson's men were beaten 3-1 by Orient in the fixture at London Road earlier in the campaign. Orient sit third in League One, just three points from top spot, while Posh are seventh, just outside the play-off positions.
Striker Conor Washington is available for selection after missing the draw with Swindon as he was cup-tied. Washington scored twice in a friendly win over St. Neots in midweek.
Nicky Ajose, who was named the Sky Bet League One Player of the Month for January, also returns. Jack Baldwin is serving the second of his two match suspension.
Striker Britt Assombalonga, who was forced off the field with an eye injury against Swindon, should be fit after treatment this week.
Boss Ferguson said: 'Leyton Orient have had an excellent campaign and they are up there for a reason. They haven't changed their style since we played them at London Road. It should be a good game and it is one of those fixtures that if things go well, could re-start our season.'
Posh had sold just under 700 tickets for the fixture by 10am on Friday morning. The game is not all-ticket for Posh fans.
